CREEKSIDE ELEMENTARY
PARENT VOLUNTEERS

Thank you for your interest in volunteering!  Before you volunteer on campus (in any capacity) or attend a field trip, you must complete a Megan's Law background check, submit a signed Code of Conduct, give us the results from a current, negative TB test and provide evidence of being fully vaccinated for Covid-19.   Please see below for more specific information regarding the required forms.  JOIN THE PTA TO STAY INFORMED.

1.  Megan's Law Form
Submit this form in person, with your Driver's License, to the Creekside main office.  Please note that it takes approximately two weeks to process this application; we encourage you to submit this well in advance of any field trips to ensure time to complete the background check.  


2.  Code of Conduct
Please read, sign, and submit this form to the main office when you drop off your Megan's Law paperwork.  

3.  Vaccination Card for Covid-19
Evidence of being fully vaccinated by providing a copy of COVID-19 Vaccination Record Card (issued by the Department of Health and Human Services Centers for Disease Control & Prevention or WHO Yellow Card) which includes name of person vaccinated, type of vaccine provided and date last dose administered).

Level II Volunteers (Check the most recent PUSD Board of Education Covid-19 Protocol Guidance)
A Level II clearance is required for volunteers that are not being supervised by a staff member and are working alone with a group of students or one-on-one with a student.  Volunteers need to be fingerprinted to be at this Level.  The teacher/staff member will determine if  Level II clearance is required.

 

  1. Obtain the Level II approval paperwork from the school office.

  2. The Level II approval form must be taken to the District Office receptionists ONLY on:    Tuesdays   10:00 a.m. – 11:30 a.m.  and Thursdays     2:00 p.m. – 3:30 p.m.

  3. Please take $20 cash or check made out to PUSD with you to the District Office.  This is the volunteer’s payment portion. The District pays the remainder of associated costs.

Retirees
Seniors Helping Our Kids (SHOK) is an all-volunteer organization that trains and places nearly 200 retirees a year in PUSD schools. Most serve as one-on-one reading tutors for children in grade 1-3, following the OASIS Tutoring program. Others are classroom or library helpers. If you are a retiree and interested in this opportunity, check out http://www.SHOKprogram.org. They will help you get all the paperwork together to participate as a volunteer.
